For 2008, all players who did not complete an Old Timer League waiver in 2007 will be required to complete a MN Senior Soccer League player waiver form for 2008.  Print the player waiver using the link below, and give the completed waiver to your manager prior to participating in any MN Senior Soccer League match.  You will be asked to initial each paragraph and sign where indicated at the bottom, in the presence of your manager (witness). 

Managers will be required to keep these on file, as well as provide a copy to the Commissioner for each player listed on their roster.

This player waiver will be required only once.  It will apply to future seasons if you continue to participate in the MN Senior Soccer League.  You will not need to do this again next year once you've signed the waiver.

The Board apologizes for this extra bit of paperwork, but it keeps the NSC and our insurance company happy.
